Why are we in Pain?

Before we can answer this question, we need to understand how the body interprets and handles pain. The pain impulse begins a the point of an injury for example a cut or burn, a pinched nerve, bruised muscle etc.

Once the impulse starts, it triggers a number of bio-chemicals to be released at the site of the injury. Some of these bio-chemicals are histamine, bradykinin, prostaglandin, and Substance P. Each of these has one or more effects on the body. And many of these bio-chemicals are inflammatory — that is they cause the injury site to swell up.

Inflammation is actually a defense mechanism for the body. Inflammation serves to bathe the injury in healing fluids and acts as a cushion to protect against further injury. However. if the inflammation is prolonged or out of control, it can cause destruction. This is what occurs in arthritis where the inflammation actually destroys the joints. Also, inflammation can serve to compound problems by actually causing pain itself. This explains how an injury causes the body to release a number of biochemicals that can cause inflammation. Another action of these biochemicals, though, is to stimulate the nerve fibers of pain, the C fibers. (The body actually has three types of nerve fibers: A fibers. B fibers and C fibers. The main fibers which transmit the pain impulse are the C fibers.) Histamine, bradykinin, prostaglandins and the other bio-chemicals are actually the stimuli that cause the pain impulse to begin.

Types of Pain Relief – At the site of an injury, whether the problem is pain or inflammation, the pain impulse can be interrupted by:

Decreasing the levels of the “pain” bio-chemicals

Or by blocking the nerves of pain – the C fibers.

With that in mind, it would make sense to use a painkiller that can do both of these. Aspirin and NSAIDS (non-steroidal anti-inflammatory drugs), such as ibuprofen and Motrin decrease the prostaglandins. This can result in decreased pain and inflammation, especially if the prostaglandins are the main causes. However, aspirin and NSAIDS do not directly affect the other pain chemicals and do not affect the pain nerve, the C fiber.

Narcotics, such as Darvon or codeine, have no known effect on either the “pain” bio-chemicals or the pain nerves. Acetaminophen (Tylenol) also has no significant effect on these at usual doses. And in fact, the way acetaminophen works is not actually known. But we also know we can interrupt the pain impulse away from the injury site, at the spinal cord. If our pain killer could also decrease the release of (or deplete the C fiber of) Substance P, the pain impulse would be blocked at the spinal cord level. Aspirin and NSAIDS have no known effect at this site. Narcotics and tricyclic antidepressants, such as amitriptylene or Elavil, on the other hand, actually can block the release of Substance P and stop transmission at the spinal cord level, but once again have no value in decreasing the levels of the bio-chemicals of pain. Torcetrapib and Rimonabant-Acomplia: Promising Next-Generation “Good” Cholesterol Raising Drugs

Based on compelling evidence that raising high-density lipoprotein cholesterol levels (HDL-C, also known as “good” cholesterol) helps halt atherosclerosis, pharmaceutical companies are racing to produce the next generation of medications to replace the ineffectual ones currently on the market. Niacin, the current drug of choice, increases HDL-C by only 15% to 35%, and then only at daily doses three grams or higher, increasing the chance for unpleasant side effects.

Today at the 33rd annual VEITHsymposiumв„ў, Dr. Russell H. Sampson, former Associate Professor of Surgery at the Albert Einstein College of Medicine (Bronx, NY) and current President of Mote Vascular Foundation, Inc. (Sarasota, FL), provided a comprehensive overview of the current state of good cholesterol-raising drugs, including two of the more promising ones, Torcetrapib and Rimonabant-Acomplia.

Torcetrapib (discovered and developed by Pfizer), is a cholesteryl ester transfer protein (CETP) inhibitor. In theory, by blocking CETP it should be possible to improve the ratio of HDL-C to low-density lipoprotein cholesterol (LDL-C, also known as “bad” cholesterol), thus slowing the development and progression of atherosclerosis. Early Phase 2 data published in April 2004 showed that inhibiting CETP with torcetrapib at 120 mg per day produced significant increases in HDL-C when given alone (46%), as well as in combination with Lipitor (61%). When the dosage of torcetrapib was increased to 240 mg per day, HDL-C increased by over 100%. However, patients taking 60 mg of torcetrapib with Lipitor also experienced an increase in systolic blood pressure, a drawback which may outweigh the benefit of raised HDL-C. It is currently estimated

that Torcetrapib will be in the U.S. market in 2008.

Rimonabant-Acomplia (discovered and developed by Sanofi-Aventis) is a CB1 endocannabinoid receptor antagonist. CB1 blockers act on the endocannabinoid system, a system controlling energy and nicotine dependence, reducing the overstimulation thought to lead to obesity and nicotine addiction. In a recent study, 1,036 overweight or obese patients with blood lipid disorders were randomly placed in one of three groups (placebo vs. 5 mg or 20 mg per day of Rimonabant-Acomplia). After one year of treatment, patients receiving 20 mg per day of Rimonabant-Acomplia lost an average of 20 pounds, compared to only 5 pounds for patients on placebo. Further comparisons between the placebo and 20 mg per day Rimonabant-Acomplia group showed that the latter patients enjoyed significant improvements in waist circumference and HDL-C and triglyceride levels, all important factors in maintaining heart health. The drug was said to be well tolerated, with only mild and transient side effects like dizziness and gastrointestinal distress. The drug is currently only approved for sale in Europe, with a date for U.S. availability still uncertain.

Dr. Samson said, “We are about to see a whole new generation of medications to help control atherosclerosis the leading cause of death and disability in the USA and most industrial nations. Current medications may halt or slow down progression, however, it is predicted that these medications may revolutionize treatment by actually reversing this disease that is responsible for heart attack, stroke, and amputations. I am extremely excited by the prospect.”

On November 2, 2009, the United States Court of Appeals for the Eighth Circuit issued its long-awaited decision in Scroggin v. Wyeth (Case No. 08-2555). The decision gives the green light for federal trials on Wyeth’s and Pfizer’s hormone therapy drugs. In affirming judgment on a jury verdict of compensatory damages, the federal appellate court rejected the defendants’ claims that plaintiffs like Donna Scroggin cannot prove that hormone therapy caused their breast cancer. It also found that the statute of limitations began to run from the announcement of the startling results of the Women’s Health Initiative study (“WHI”) in July, 2002.

Erik Walker of Hissey Kientz, LLP in Austin, Texas handled the appeal for Ms. Scroggin, writing her briefs and arguing to the court. Walker lauded the court’s decision. “The opinion reveals the court became well-versed in the complex issues presented in this litigation. The decision finally forecloses Wyeth’s cynical claim that even though its drugs caused countless women to develop breast cancer, Wyeth can never be held accountable because no woman can prove that her particular cancer was caused by the drugs.”

Donna Scroggin took Wyeth’s Premarin (estrogen) and Pfizer-subsidiary, Upjohn’s Provera (progestin). She then took Wyeth’s Prempro, which combined estrogen and progestin (E+P) into a single pill. After 11 years using the products, Ms. Scroggin developed hormone-dependent cancer in both of her breasts, compelling a double mastectomy and debilitating treatments. A jury awarded Ms. Scroggin $ 2.75 million in compensatory damages. The jury also found that Wyeth and Upjohn’s negligence reflected reckless disregard to women’s health, awarding nearly $ 20 million in punitive damages against Wyeth and nearly $ 8 million against Upjohn. The district court rejected Wyeth’s attacks on the compensatory award but set aside the punitive awards. The Eighth Circuit found there was sufficient documentary evidence of reckless disregard to warrant a new trial on punitive damages against Wyeth.

Jim Morris, who tried the case, looks forward to the new trial: “We appreciate the opportunity to once again present evidence of Wyeth’s conscious disregard for the rights and welfare of Ms. Scroggin to a jury. Most juries find Wyeth’s conduct to be reprehensible and we expect Arkansans to be equally incensed yet again.” Walker said the decision is great news for breast cancer victims throughout the country. “Studies show that E+P has caused over a quarter of a million women in this country to develop breast cancer. Only a fraction of those women have filed suit. This decision ensures that these women may continue their pursuit for justice.”
